FORMAT FOR BAN APPEALS - READ BEFORE POSTING.

When posting a ban appeal, be sure to include:
1) Your GUID (this is displayed in one of the Settings menus within the game)
2) Your IP address in case of an IP ban
3) Your UTAN Ban ID in case of a UTAN ban. If you have been UTAN banned, this is displayed on a large ban screen when you try to connect to the server.
