I didn't read ALL the topic but I've something to say: aren't you too "strict"? Maybe it's only in my country, but we often "friendly offend" someone, calling him idiot, stupid, fool, ecc ecc but never with hatred behind the words..

In my country this can happen only between a very good friends who know each other well and only by voice talk when phrase meaning is showed by intonations.

Yes, in mine too but sometimes it happens also between two guys who have met each other just 5 minutes before.
But i you are right, the problem is the intonation.. maybe a smile could help, in the chat.

I agree with DeathBlade.

As long as the insults aren't personal, e.g. racism, making fun of a mentally challenged family member etc. I really don't mind. I usually just mute people who annoy me from my personal chat.

This game is an M rated game for a reason, cursing is a natural part of competition. But racism and 'ad hominem' attacks are definitely not. My 0.02.
